Section II: Select the type of leave 1) University Withdrawal - Permanently leaving the university. You will no longer be enrolled. 2) Leave of Absence - Temporarily taking a break, with the intention to return within 4 semesters. 3) Military Leave - Must submit this form together with the official military enlistment letter (입영통지서). 4) Medical Leave - Requires submission of supporting medical documentation.
A student's signature is required.
Date: Students must fill out the date on which they submit this form. Any discrepancy between the written date and the actual submission date will result in the form being rejected and requiring revision.
